Self service mobile app for patients and their care takers

Problem Statement

Older demographics often struggle with new technology. As health care evolves more and more tech is introduced. There is also a gap between the information a provider has and what the patient can access that limits a patient's ability or a care taker to provide optimal coverage outside of the doctor's office.

Output

This was an exploratory project aimed and ideating around what a patient health portal could look like. I set out to design an extensible UX where users could easily pair their device with at home health monitor devices and smart watches.

Leveraging the data provided from their devices as well as information pulled from their Electronic Health Record I designed a comprehensive mobile application that tracked their daily activities and gamified the experience to drive user engagement.
